30215QLD - Construction Industry General Safety Induction

Welcome to the Axial online Blue Card Course for Construction Workers.

New Rules in 2008

Before enrolling in the course you should be aware of the new rules that apply to the online version of the Blue Card, covering proof of identity. To comply with these rules, Axial requires that you must send us the following, once you finish the course:

  1. a certified copy of your drivers license
  2. a Statutory Declaration (provided), stating that it was you who completed the course and the assessment, witnessed by a JP/C.Dec.
  3. the original printout of the certificate issued online, when the assessment is successfully completed. This needs to be signed by you.

What is the difference?

  1. You will not be able to use the computer-printed certificate in order to work. You will have to wait until the completed and laminated card is sent to you (within 5 working days of verification).
  2. You will need to send 2 EXTRA documents (provided) in your application.
  3. You will need to get a Justice of the Peace (JP) or Commissioner for Declarations (C.Dec) to sight and sign/witness these 2 documents.

Finding a JP

To find a JP/C.Dec near you, use the Qld Government search facility at http://www.justice.qld.gov.au/jps/finding.htm

Not in Queensland?

The statutory declaration provided is only relevant for Queensland. If you live elsewhere you must provide a statutory declaration that is relevant to your State or Country AND get it witnessed (as well as your identification) by an appropriate authority for where you live.

To find a statutory declaration template for your State, go to the following site: https://www.docdownload.com.au/document/group.psp?group=legal,12918
